How To Get Your Own Quote

We have two different plans.

If your child is less than 2 years of age, you must use Plan 2.

If your child is between 2 years and 18 years old, you may use Plan 1 or Plan 2

We would be more than happy to have you call us and we will fill out the enrollment form for you. It takes less than 5 minutes.

What is the difference between these two plans?

Plan 1

Advantages:

  • Less expensive than Plan 2

  • Can be taken for up to 12 months (as opposed to 6 months) before it has to be renewed

  • Unlimited office visits for a $50 Co-pay

  • Can be renewed with only a 24 hours lapse in coverage.

Disadvantages

  • Child must be at least 2 years of age

  • Higher out of pocket total than Plan 2 ($5,000 versus a maximum of $2,000)

Plan 2

Advantages:

  • Can be used for children from 30 days to 18 years

  • Lower total out of pocket costs in the event of a hospitalization.

Disadvantages

  • Slightly more expensive than Plan 1

  • You must have a 30 day lapse between renewal (can be covered by Plan 1 for those 30 days)

Plan 1 - Ages 2 years old - 18 years of age

Even though the button you will see says "Quote and Apply", I can assure you that it will first present quotes to you. You need to go through a second step to actually apply. There is no obligation to do so.

You will now see a button that says "Click Here to Quote and Apply" - click on it.
Select your state from the drop down box. Enter in your zip code and the child's gender where it says Applicant's Gender.
To the right of the gender box is a drop down list where you can select their age. If this is for one child, that is the only information needed.

Below this information you get a choice of selecting Monthly Payments or a Single Up front Payment.

If you check monthly payments you can get up to 12 months of coverage at a time and pay on a month to month basis. If you check Single Up front Payment you will get a discount (about 20%) but you must pay for the number of days you want the plan in advance.

There are no refunds for this option. Check on of the boxes and then click Quote Now.

If this plan is for more than one child, use the above procedure for the oldest child and to add any additional children, enter the number of additional children in the box that says Number of Dependent Children. If there are three children for example, this box would show the number 2 and the oldest child being the applicant.
You are now on the page that shows you all the quotes. Some states will display both 6 month and 12 month rates, while other states will only show 6 month rates.
Click on the line that has the plan deductible you desire. It will expand and show you a button that says Apply Now. There is also a place for a check mark that says you are the parent or legal guardian. Do not forget to check it or you will not be able to proceed.
Once you click the Apply Now button you will be taken through the application process. It is very simple. If you need assistance, just call us.

 

Plan 2 - Ages 30 days - 18 years of age

To begin a quote for Plan 2 you should click on this link: Quotes for Child Only Health Insurance at least 30 days old.

- Enter in your zip code.
- There will be a default start date but you can change it.
- Where it says Primary select the gender and fill in the date of birth.

If this plan is for more than one child, use the above procedure for the oldest child and to add any additional children, enter the number of additional children in the box that says Number of Dependent Children.

If there are three children for example, the Number of Dependents box would show the number 2 and the oldest child being the applicant.

You are now on the page that shows you two columns for quotes. The column on the right allows you to adjust the deductible and coinsurance.
When you change the deductible or coinsurance, the rates will change. There is also an option at the bottom for an accident plan. This is an option that pays out for an emergency room for accident or injury without meeting the plan deductible.
Once you know which plan deductible you want, click the blue Select button and you will be taken through the application process. You will be shown a Plan Summary and you will see the Apply Now button. It is very simple. If you need assistance, just call us.
